Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who we are

The Crane Center, LLC (“The Crane Center,” “we,” “us”) provides counseling and ketamine therapy, concierge primary care, and women's health and aesthetics at our Destin and Panama City Beach locations. As a health care provider, we create and keep medical records and other protected health information (PHI) about you. This notice explains how we use that information and your rights under the Health Insurance Portability and Accountability Act (HIPAA).

How we use and share your health information

We may use and disclose your PHI without written authorization for:

  • Treatment. To provide, coordinate, or manage your care — including with other providers involved in your treatment.
  • Payment. To bill and collect for services, verify coverage, and obtain prior authorization when needed.
  • Health care operations. Quality review, staff training, licensing, accreditation, business planning, and running the clinic.
  • Appointment reminders. We may text or call you at the number you give us, including (844) 909-4156.

We also share information with vendors who help us operate (website hosting, electronic forms such as new-patient paperwork, email, and analytics). They receive only what they need to do their work and are required to protect it.

Other uses that do not need your authorization

Federal and Florida law allow or require us to share PHI in limited cases, including:

  • When required by law, court order, or lawful subpoena
  • Public health activities (for example, reporting certain diseases)
  • Reports of abuse, neglect, or domestic violence as required by law
  • Health oversight activities (audits, investigations, licensure)
  • Law enforcement purposes as permitted by law
  • To prevent or lessen a serious and imminent threat to health or safety
  • Coroners, medical examiners, and funeral directors as needed
  • Workers' compensation and similar programs

Uses that require your written permission

Most other uses and disclosures — including most marketing and any sale of your PHI — require your written authorization. We do not sell your health information. You may revoke an authorization in writing at any time; that does not affect uses we already made in reliance on it.

Psychotherapy notes, where we keep them separately, are used and disclosed only as HIPAA allows, generally with your authorization except in limited situations such as to defend against a legal action or when required by law.

Your rights

You have the right to:

  • Request a copy of this notice (paper or electronic) at any time
  • Inspect and obtain a copy of your medical record, with limited exceptions under HIPAA
  • Ask us to amend your record if you believe it is wrong or incomplete
  • Request a list of certain disclosures we have made (an accounting)
  • Ask us to limit how we use or share your PHI. We are not required to agree, except that if you pay a service in full out of pocket we must honor a request not to share that information with a health plan for payment or operations, unless the law requires it
  • Ask us to contact you in a specific way or at a specific address or number
  • Be notified following a breach of your unsecured PHI
  • File a complaint if you believe your privacy rights have been violated

Requests should be made in writing using the contact information below. We may charge a reasonable, cost-based fee for copies as allowed by law.

Our duties

We are required by law to keep your PHI private, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice that is currently in effect. We may change this notice. The new notice will apply to all PHI we already have as well as information we receive in the future. The current notice will be posted on this page and available at our offices.

Complaints

If you believe we have violated your privacy rights, you may contact us using the information below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint. You will not be retaliated against for filing a complaint.

Website, forms, and cookies

If you fill out a form, complete new-patient paperwork, text us, or email us, we collect the information you send. Our website and analytics tools collect standard technical data such as IP address, browser, pages viewed, and referring URL. You can block cookies in your browser.
